  • Overview

    The Bridge is a human service agency that provides counseling services to people facing challenges. This high intensity DMH-funded residence serves six adults living with serious and persistent mental illness. This structured residence is designed to assist individuals with previous attempts at community living to learn new skills that will help them to be successful in the community and prepare them for transitions to less restrictive environments. This program offers Peer Support and utilizes Supported Employment and Dialectical Behavior Therapy (DBT) as the treatment models.


    The Bridge provides excellent supervision (both individual and group), as well as various trainings in which to obtain CEU’s, and opportunities for advancement. The Bridge is an eligible employer for the Public Student Loan Forgiveness Program and provides tuition reimbursement for continuing education. 


    The Treatment Coordinator is responsible for assisting the Clinician with developing and writing treatment and behavior plans, and will help facilitate groups. The Treatment Coordinator will be responsible for maintaining collateral contacts, assisting clinicians with assessments, and assisting with the scheduling and facilitation of Person Centered Planning meetings. The Treatment Coordinator also provides direct care to individuals, shift supervision, group facilitation, serves as a member of the leadership team, and has division on-call responsibilities.


    • Bachelor's Degree in Social Services related field preferred
    • A minimum of one to two years of experience working with adults living with serious and persistent mental illness.
    • Candidate must have knowledge of writing treatment plans and assessments. 
    • Candidate must be at least 20 years of age, have a valid driver's license, and be able to demonstrate proficiency with Microsoft Word.
    • Candidate must be First Aid, CPR, and Medication Administration Program (MAP) certified or certifiable.
