The Bridge of Central Ma, Inc. is the Lead Agency collaborating with three Affiliated Partners to form Central Community Health Partnership (CCHP). CCHP will provide Community Partner services, which includes Care Coordination and Care Management, in a new integrated health care model. Through a contract with the State, CCHP will provide Behavioral Health and Long Term Services and Supports coordination partnering with primary care physicians, behavioral health organizations, and community and social service agencies.

The Vice President of Central Community Health Partnership (CCHP) will provide leadership, management and strategic direction to the BH CP and the LTSS CP.  The Vice President will manage the Core Team members including senior clinical care managers and the administrative positions that provide services for both CPs.  S/he will report to the Executive Vice President of Service Operations at The Bridge of Central Mass, and will be accountable to the CCHP Governing Board for all operational and programmatic results.  The Vice President, CCHP will be the primary contact for EOHHS on matters related to the Community Partner programs, and will build and enhance relationships with Accountable Care Organizations, Managed Care Organizations, Lead Agencies, Affiliated Partners, Material Subcontractors, and other Community Based Organizations. 


The Vice President of the Central Community Health Partnership will:

  • Oversee the overall operation of the Behavioral Health and LTSS Community Partner Programs.
  • Provide leadership, management and strategic direction for The BH and LTSS CPs
  • Represent the program to the state, lead agencies, affiliated partners and material subcontractors, provider partners and external organizations.
  • Ensure program operates in compliance with contractual obligations, service delivery standards and adheres to policies and procedures.
  • Manage the overall BH and LTSS CP budgets and staffing requirements.
  • Facilitate program development and improvement activities based on input from Quality Committee, Consumer Advisory Boards, and performance measures.



  • Master's degree in a health, behavioral health or human services related field.
  • A minimum of five years-related managerial experience.
  • Applicable knowledge and experience in health, behavioral health, managed care and human services continuums.
  • Highly organized with strong management and relationship skills.
  • Ability to evaluate and control complex systems with multiple partners
