The Bridge is a human service agency that provides counseling services to people facing challenges. The Northboro Community Residence, Supported Housing and Stepping Out Services will provide 15 young adults (9 Residential beds, 2 supported housing slots and 4 with Stepping Out Services) with an array of out-of-home residential treatment services when needed. The services provide flexible individualized treatment, rehabilitation, and support/supervision that vary in intensity based upon individual needs. The service will progress from intensive residential treatment through a transition to a more independent living situation.
The Stepping Out service is designed to promote wellness and recovery through person centered services and supports that enable individuals to live, work and participate in their communities. These follow up services will include care coordination, outreach, transitional supports and service linkages.
Supported Housing Counselors are responsible for providing support services, transportation of individuals, medication administration, and service documentation as required. Supported Housing Counselors also provide crisis counseling to all young adults served as needed and maintain an assigned caseload.
