Overview

The Bridge is a human service agency that provides counseling services to people facing challenges. This program serves adults with intellectual disabilities and acquired brain injuries referred by the Massachusetts Rehabilitation Commission (MRC) and Department of Developmental Disabilities. 

 

The Bridge provides excellent supervision (both individual and group), as well as various trainings in which to obtain CEU’s, and opportunities for advancement. The Bridge is an eligible employer for the Public Student Loan Forgiveness Program and provides tuition reimbursement for continuing education. 

Responsibilities

CASE MANAGEMENT RESPONSIBILITIES      

  • To provide regular support, counseling and Individual Plan development and monitoring for individuals served.
  • To provide counseling, structure weekly activities, and crisis intervention on an as needed basis.
  • To provide transportation to individuals served as required.
  • To assist in coordination of skills training for individuals served and provide life skills training to appropriate individuals and groups.
  • To maintain files for individuals on caseload.
  • To prepare the assessments required for Individual Support Plan development for each individual with the Program Manager per the standards of the Bridge and the funding source.
  • To provide ongoing assessment, problem solving, skill training, supervision (e.g., modeling, prompts, assignments, monitoring, encouragement), and environmental adaptation to assist individuals with the completion of activities of daily living.
  • To provide on‑call coverage for the program as assigned.

SURROGACY RESPONSIBILITIES  

  • To serve as PCA Surrogate for individuals who require this assistance including the hiring and supervision of PCA's as Surrogate.
  • To provide support and assistance in the development of leisure and recreational opportunities, and coordination of health-care services and benefits.

           

Qualifications

  • BA/BS in a Social Service related field strongly preferred, plus or one to two years of experience or knowledge and experience working with individuals with intellectual disabilities, or acquired brain injury and/or physical challenges.
  • Valid driver's license and reliable automobile required. 
  • Eligible candidate must demonstrate proficiency with Microsoft Word.
  • Candidates must be CPR, First Aid, MAP (Medication Administration Program) certified or certifiable.
  • Candidate must be at least 20 years of age.
