  • Overview

    Adult Community Clinical Services(ACCS) are comprehensive clinically focused services provided by a multi-disciplinary team that provides services to adults with mental health needs.  Our agency  is currently seeking qualified RN candidates in the North Central Worcester area.


    1. Forge a mutually respectful partnership with agency employees, persons served and their families.
    2. Collaborate with the Integrated Team to complete a thorough physical assessment of health needs; provide referral to appropriate community physicians/health care providers as needed.
    3. Provide training and consultation to programs regarding medical issues, procedures & techniques, and health & wellness practices.
    4. Assess and provide individualized education related to person’s health and disease processes; reinforce wellness and preventative health measures.
    5. Monitor medical and psychiatric status by: observing and interviewing individuals, examining documents, and making recommendations.
    6. Cultivate mutually respectful partnerships, set appropriate limits, and maintain a helping role with people served and family members.
    7. Demonstrate respect for all individuals regardless of ability status, culture, sexual orientation, gender identity, religion, or ethnicity.
    8. Document all individual encounters and contacts made on behalf of individuals/families; development of Nursing/Health Care Plans and nursing summaries.
    9. Ensure effective implementation of Nursing/Health Care Plans through staff training, evaluation, and modification when needed.
    10. Participate in the development and implementation of the Treatment Plan as needed.
    11. Provide 1:1 support and intervention for people served as needed.
    12. Participate in meetings and communication with other providers as needed.
    13. Develop and implement, in conjunction with staff, health and wellness and symptom management groups and interventions.
    14. Review clinical records on a regular basis to ensure adequate services and supports are provided, including any applicable medical interventions, tests, or preventative care as necessary.
    15. Administer and monitor individuals' medical needs in accordance with orders by prescribing physicians as needed.
    16. Provide oversight to the Medication Administration Program and ensure compliance with applicable regulations.
    17. Facilitate individual access to health care and community resources as appropriate to meet the needs of individuals in the community.
    18. Cultivate ongoing relationship with other health care providers to provide consistency in prescribed treatment procedures, techniques, etc. including follow-up or ongoing care following a hospitalization as needed.
    19. Identify and provide staff training in collaboration with Integrated Team members on an ongoing basis.
    20. Participate in the monitoring and corrective action activities of the agency’s Health and Safety Committee.
    21. Attend, participate in, or facilitate meetings/trainings and supervisions as assigned.
    22. Advocate for and with People served to physicians and other service providers.
    23. Participate in on-call rotation for the nursing department.
    24. Perform other duties as assigned.




    Registered Nurse licensed in Massachusetts. Experience with adults with mental health and psychiatric disorders preferred. Excellent assessment, documentation, and verbal communication skills required. Valid driver’s license required.
