The Bridge is a human services agency providing evidence-based treatment and services for those with mental health challenges, developmental disabilities, acquired brain injury and complex family problems. The Bridge provides excellent supervision (both individual and group), as well as various trainings in which to obtain CEU’s, and opportunities for advancement. The Bridge is an eligible employer for the Public Student Loan Forgiveness Program and provides some tuition reimbursement for continuing education.
A new 6 bed mobile respite and site based respite program is opening. It will be located in Gardner, Ma. In addition to providing on site mobile respite services, the program will also work with individuals in the community providing short term support services. These services are meant to identify and asses the immediate needs of the individuals referred by DMH, and provide short term support in achieving their more immediate needs
Responsible for physical assessments, maintenance of health records and planning and implementing educational groups with individuals served and staff related to each individual’s health and disease processes. Assist staff with the implementation of the Medication Administration Program (MAP) and provide training to staff to meet the regulations. Support and actively participate in agency-wide wellness initiatives. Assist staff with appointment scheduling, follow-up with physicians, and hospital discharge planning. Complete required documentation in a professional and timely manner. Eligible candidates must have a solid medical-surgical background due to many of our individuals having co-occurring diagnoses. This nurse will also coordinate care, attend medical appointments with the individuals, review medications, and work directly with the individual's served.
Eligible candidate must be a registered nurse (RN) and BSN is preferred. Excellent written and verbal communication skills and ability to collaborate with multi-disciplinary team members required. Valid driver’s license and reliable vehicle for travel between programs and providing transportation is required. Position requires the ability to work flexible hours. No weekends are required.
