  • Overview

    The Licensed Practical Nurse works in close collaboration with the Registered Nurse to ensure the medical needs of the person supported is met.  This position involves acting as a liaison between the program and other stakeholders including residential program, family members, and other medical providers.  The LPN can expect to provide 1:1 support to the person served while at the program and while out in the community.  This 40 hour position is first shift Monday-Friday.


    1. Develop and implement the Day Hab Service Plan with other members of the interdisciplinary team.
    2. Teach skills and create opportunities for practice, including data collection.
    3. attend to all personal hygiene routines/needs
    4. Identify needed services.
    5. Work collaboratively with other staff to organize, monitor and implement the daily routines of the group. 
    6. Provide support within the program and in the community.
    7. Monitor medical needs in accordance with orders by prescribing physicians as needed.
    8. Administer medications according to physician orders.
    9. Perform medical procedures as prescribed.
    10. Assist with maintaining emotional and behavioral stability, apply SOLVE interventions as needed.
    11. Accompany the individual served to medical visits as needed.
    12. Advocate for the individual served with physicians and other providers as necessary.
    13. Work in close collaboration to the RN maintain medical records and proper documentation.
    14. Attend in-services and trainings as assigned.
    15. Acquire and maintain all required certifications.
    16. Support all agency policies and procedures. 
    17. Provide coverage from the individual's home; provide assistance as needed during transportation to and from the day program.
    18. Provide and attend to all needs during program hours, approximately 7:00am-3:00pm
    19. Provide LPN supports in the home when individual is unable to attend ALC or program cancelled.
    20. Requests fro cation require at least a 2 week notice to ensure proper coverage.
    21. Ensure physical plant maintenance, included but not limited to health & safety and cleanliness of the program.
    22. Perform other related duties as assigned.


    Current Massachusetts Registration. At least 1 year experience in a health care setting working with persons who have developmental disabilities. SOLVE certified (within 6 months of hire).
