  • Overview

    The Bridge is a human service agency that provides counseling services to people facing challenges. Continuum services are community-based wraparound services that are designed to maintain youth within their homes and support families as the primary caregivers. This includes in-home family treatment, parent support, youth mentoring, youth and family outreach, care coordination and linkage with both formal and informal community resources and supports. The continuum will also provide long-term and short-term out-of-home care for youth who cannot be maintained safely at home (i.e., group home, pre-independent living, intensive foster care and respite).


    The Bridge provides excellent supervision (both individual and group), as well as various trainings in which to obtain CEU’s, and opportunities for advancement. The Bridge is an eligible employer for the Public Student Loan Forgiveness Program and provides tuition reimbursement for continuing education. 


    The Outreach Counselor will provide an average of three to four hours of direct support weekly to each assigned family. Services will be tailored to meet the unique needs of each youth and family served, providing opportunity to maximize their ability to spend time together at home and in the community. Responsibilities include:

    • To provide advocacy and outreach services to assigned caseload of youth and their families, in individual or family's home and community.
    • To monitor Individual Treatment Plans (ITP) for all youth and families that supports their capacity for growth and change.
    • To establish and set appropriate limits and boundaries as a practitioner in order to assure the safety and protection of the family members and maintain the helping role of the practitioner.
    • To provide support and structure to individual and families while monitoring their progress towards completion of service goals.
    • To transport individuals in a safe manner in matters related to service delivery.
    • To provide on-call emergency coverage as assigned.



    • Bachelor's Degree in a Social Services related field preferred.
    • Two years of experience with youth and families.
    • Valid driver's license and reliable vehicle.
    • Proficiency with Microsoft Suite including Word.
    • First Aid and CPR certified or certifiable.
